    Thank you for writing this article. I am a pharmacist working at a Family Health Team, and collaborate with my family physician colleagues around antithrombotic therapy. I appreciated so many thoughtful considerations in this piece that speak to how we communicate and what we communicate to patients around risks and benefits of pharmacotherapy. In my experience, a collaborative practice model between pharmacists and family physicians around high-risk medications such as anticoagulants can be valuable to support patient-centred decision-making that is founded on evidence-based recommendations.
