Home-based Primary Care in Canada (Nov. 2022, CFP) (1) by Sawchuk, Condon and Stewart are an important article on the most compelling issue now facing Canada's health care system. That is, how are we meeting the needs of the frailest and complexly co-morbid of our clients.
Their qualitative analysis of 5 Practices focused on the home-bound is elucidating. Initiating factors, and the barriers and facilitators to implementation were most significant.
(2). Euphemistically/diplomatically, major barriers are referred to as "limited health system leadership support", and "obstructive.... systems-level policy".
This I believe refers to lack of creativity and leadership from our Provinces and regional health authorities. Our Provincial and National Doctors organizations, e.g., OMA, CMA, Doctors Manitoba et.al. also need to lead and lobby for these much-needed changes in primary systems delivery and remuneration.They conclude that " adopting these models....in the community is key to the LONG-TERM SUSTAINABILITY of our health care system" (my italics). Connecting some dots, excessive wait times in Canada's E.R. departments, both time to see a provider, and time for admission to a hospital bed from the E.R. are inextricably linked to insufficient home care and lack of focused community-based primary care options for these hospital-bound people (3, 4).
