Moreover, the fact that the marketing authorization for phentolamine, an antidote used in other countries for epinephrine-induced emergencies [19,20], is only valid in France for oral surgeries [21], thus significantly hampering its accessibility, especially in the hospital setting. Consequently, an anesthesia consultation is still required, at least in our teaching hospital, to circumvent the absence of phentolamine, even for surgeries performed with WALANT, because axillary brachial plexus block (ABPB) is another rescue technique to reverse vasospasm [22–24].

Pale and severely painful digits have frequently been reported (86%, 56.1% respectively),49 but the psychological implications of the injury, which we feel are an important aspect of the patient’s suffering, were under-represented and only reported in one case.50 We propose a simple treatment algorithm (Fig. 2) for surgeons and emergency physicians to follow after a digital injury with an AAI device.

2012, Journal of Hand SurgeryCitation Excerpt :The second case involved surgical debridement of a finger infection in a patient with scleroderma and Raynaud phenomenon and the authors concluded that the necrosis was likely caused by the infection, pre-existing microvascular damage, or excessive necrosis caused by the laser ablation rather than the lidocaine. Several case reports have been published on accidental injection of high-concentration (1:1,000) epinephrine into the hand and fingers.9–19 In most of these cases, the effect of epinephrine was reversed using phentolamine.
